Amazing Home Care is actively seeking motivated and experienced Full-Time Registered Nurses to perform UAS, home care initial and reassessment visits for our Licensed Home Care Agency in the boroughs of Brooklyn, Queens, Manhattan and the Bronx. UAS certification preferred. Bilingual speaking a PLUS.

Job Summary:

Demonstrate proficient skills to admit and reassess home health patients in regards to physician ordersCoordinate and supervise HHAs in the delivery and discharge of patient careInitiate home care visits and reassess patient needs and progress on a regular basisInitiate the plan of care under doctor’s ordersObserve, assess and document all patient progressNotify physician and other personnel of changes in conditions of patientsEducate patients and caregivers on disease process, medications, plan of care, and individualized treatment plan as well as in-home health care

Qualifications/Educational Requirements:

Must have an active New York State RN LicenseMust be vaccinated against COVID-191+ years of experience working in a Hospital & Home Care settingWork positively and favorably with patients, families, and staffExcellent communication and time-management skillsAbility to travel to multiple patients per dayBilingual in English & Spanish/Creole/Cantonese/Mandarin/Russian a plus
