40 hours – $1,890.00Total Travel Package Weekly(over 75miles)

$640.00 Taxable – Weekly Taxable Wages

$800.00 – Weekly Lodging Allowance

$450.00 – Weekly Meal Allowance

*Based upon qualifying for position and allowances. Totalpackage is inclusive of taxable wages and allowances.

Social Work/ Case Management

2 years’ of RECENT acute hospital experience in discharge planning

Master’s in Social Work – Required

8am-4:30pm

The Unlicensed Social Worker/Counselor/Mental HealthClinician provides counseling to patient’s in a variety of settings includingmedical facilities, hospitals, clinics, learning centers and otherorganizations that are in need of assistance.

Essential Duties and Responsibilities:

Provides necessary counseling as required for thepatient/client Assesses the patient/clients’ psychosocial needs, andevaluate the progress with the care team and/or supervisor Collaborates with all disciplines to plan and evaluate goalsfor each patient/client Focuses on assisting patients/clients through the dischargeprocess of both in and out-patient treatment facilities as applicable Utilizes a variety of therapy techniques and treatment plans Facilitates patient progress and referrals with otheragencies Adheres to all Amergis and Facility policies andprocedures  Observes and reports patient/client progress per facilityprotocol Performs other duties as assigned/necessary

Minimum Requirements:
