JOB DESCRIPTION:

Minimum Qualifications:

Must possess a Master's Degree in an appropriate social service discipline. Must have experience with mental health and/or co-occurring issues. Ability to develop supportive relationships with persons with legal, mental health and co-occurring abuse issues while maintaining appropriate therapeutic boundaries.  Possess knowledge of mental illness and recovery including the ability to collaborate with community partners.  Valid Florida Driver's License with a safe driving record. Competency with computer systems and concurrent documentation.

Preferred Qualification:

Bilingual English/Spanish is preferred but not required.  Knowledge of community resources a plus. Telehealth experience.

Special Skills, Knowledge and Abilities: 

Excellent verbal and written communication skills. Understanding of populations affected by mental illness, proven skills in networking among diverse agencies, and strong planning and personnel management abilities. Must value cultural differences, be able to motivate others as well as work as a team member, and be genuinely interested in working with populations experiencing mental illness.

Essential Functions: 

Provide crisis intervention in the community.

Ensure development of Individual Treatment Plans (ITPs) to include priorities within the treatment team.

In conjunction with the team, responsible for developing, writing, implementing, evaluating and revising overall treatment goals and plans per on-going assessment of needs with input from the client and family.

Conduct on-going assessment of behavior, mental status, signs and symptoms of mental health disorder, physical and dental health, substance use, and educational, social, legal, financial, housing, developmental and family functioning to specialize case management needs. 

Provide supportive and educational counseling services (individual, family and group) to develop daily living and coping skills.

Assist clients with developing natural support systems.

Input written documentation into Avatar in a timely manner for all services rendered.

Refer, advocate for and link client and family for additional services as identified.

Complete all required agency trainings as to maintain 100% compliance.

Perform crisis intervention to include use of and understanding of Baker Act.

Work Environment/Physical Demands

This position operates in a professional office environment, judicial building and in community based settings.  Sits, stands, bends, lifts and moves intermittently during work hours.  Is subject to frequent interruptions.  Is involved with clients, family members, court and county personnel and government agencies.

Position Type, Expected Hours of Work and Travel: 

This is a full time position; must be able to work a flexible work schedule to include early morning and evenings.  On call rotation and travel may be required. 

Position may be available but not limited to any of the following programs:

Housing Outreach Team (HOT): The target population is homeless individuals and families that are addressing behavioral health, mental health, and substance use disorders.

Assisted Outpatient Treatment (AOT): The target population is high need/high utilizer individuals who are addressing both behavioral health and mental health concerns.

Harm Reduction (WISH): The target population is individual with opioid disorder or other identified substance use disorders who are at high risk of overdose.
