Case Manager
Per Diem
Day Shift Mon-Friday
8am - 4pm
one weekend shift a quarter required
The Case Manager incorporates the case management process of assessment, planning, implementation, coordination, monitoring and evaluation into daily practice to manage the patient's broad spectrum of needs across the continuum of care. The Case Manager applies clinical expertise and core knowledge domains of case management into daily practice. Data is interpolated from patient/ family assessments, the interdisciplinary care team and medical record to evaluate the patient's inpatient plan of care, progress along clinical pathways and care guidelines and discharge plan. Collaborates with the patient/ family, interdisciplinary care team, medical staff, payers, and post discharge providers to facilitate and coordinate patient specific evidenced-based care. Patient care is transitioned through communication and appropriate referral to the next provider of care.
Requirements
The qualified candidate will possess a minimum of 2 to 3 years clinical nursing experience coupled with knowledge of case management, with emphasis on discharge planning. The ability to problem solve, multi-task and manage projects independently is a must. Knowledge of regulatory and insurance standards and protocols as well as proficiency in utilizing evidence based guidelines. Current NYS RN licensure and Bachelor's degree in Nursing or related field is required. CCM certification highly desirable.
