Qualifications and requirements
Applicants should have a current state RN license and possess a bachelor’s degree or associate degree from an accredited nursing school. Additional requirements include:

Previous experience in a physician’s office or similar setting. Minimum of 2 years of nursing experience, preferably in cardiac or vascular care

Department/Unit Summary

The RN Navigator demonstrates performance consistent with the mission, vision and values of the organization. They remain flexible to changing systems - is expected to demonstrate quality, effectiveness in work habits, clinical practices, and treats staff, physicians, patients and families with consideration and respect. As allowed by their licensure, prepares patients and assists during examinations, treatment, procedures, patient navigation, clinical support, limited surgical support function and/or diagnostic testing.

Maintain current knowledge on vascular and cardiac therapeutic regimens. Maintain current knowledge on side effects and symptoms related to vascular and cardiac therapeutic regimens. Maintain current knowledge base on complications associated with surgery for vascular patients. Assist physicians in the health maintenance of patients in a complex disease state. Patient navigation and care coordination for vascular clinic patients. Assist doctors in performing physical tests and prescribed tests. Help to conduct and interpret laboratory test results and help to interpret results. Call patients with abnormal results and treatment plans in a timely fashion. Educate and inform patients of the best disease prevention and control methods. Work with doctors and other departments whenever a patient is undergoing surgery. Monitor/manage, screen telephone requests for assistance by urgency of the existing patient base. Assist providers in refilling chronic medications per protocols. Interview and assess patients for medical interventions in the office and on the telephone. Initiate medical interventions based on a symptom-focused protocol or guideline. Advise and give professional and moral support to patients suffering from heart conditions, who may not understand the steps to take to manage their conditions. Educate and monitor patient compliance regarding medical interventions. Monitor the psychosocial and emotional needs of patients for outside referrals and additional care. Assist the physician with the medication management of the existing patients. Support medical assistants, perform quality improvement audits and tasks within the clinic. Assist in the pursuit of specialty drugs and/or prior authorizations on behalf of the patient. Manage specialty drug requisites including patient interviews, record keeping and reporting. Prepare and maintain accurate medical records.
