EAC Network, a leading not-for-profit social service agency dedicated to helping over 62,888 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full time Veteran Case Manager to join our Queens Misdemeanor Treatment Veteran’s Court Program. Competitive Pay:$28 per hour at 35 hours per weeka Schedule: Monday to Friday, 9:00 AM to 5:00 PM. Location: Kew Gardens, NY Reports to: Program Director Why This Role is Perfect for You: As a Veteran Case Manager, you will play a pivotal role in transforming the lives of veterans by providing comprehensive case management and support within a structured and compassionate environment. What You'll Do: + Assess & Support:Conduct thorough assessments, refer veterans for further evaluation if needed, and develop treatment plans to present in court. + Coordinate Care:Arrange and implement placements into appropriate treatment programs and assist clients in securing benefits such as public assistance, SSD/SSI, housing, and medical care. + Maintain & Monitor:Keep detailed client records, make site visits to treatment agencies, and ensure ongoing contact with your caseload. + Facilitate Groups:Lead Seeking Safety groups and assist with supervised toxicology testing as required. + Assist & Escort:Provide support by escorting clients to treatment facilities and collaborating with treatment and mental health systems. What We’re Looking For: + Qualifications:Bachelor’s Degree in Psychology, Behavioral Science, or a related mental health field. High School Diploma with substantial experience in substance abuse and mental health will also be considered. + Skills & Knowledge:Working knowledge of treatment programs and Veterans Administration services, excellent communication skills, and proficiency in computer applications including Microsoft Office and databases. + Experience:Previous experience in case management or working with veterans is a plus.
