Veteran Service Care Coordinator

This trusting relationship enables the Veteran Service Care Coordinator to serve as a liaison among Marshfield Clinic Health System (MCHS), Veterans Affairs, social services and the community to facilitate access to services for Veterans. The Veteran Service Care Coordinator assists with screening Veterans for unmet medical needs, social determinants of health, clinical quality indicators, mental health conditions, and connections to VA services to optimize care coordination and improve clinical outcomes. This individual functions as a member of the healthcare team by providing basic clinical, clerical, and technical support to assist Veteran patients. **JOB QUALIFICATIONS** **EDUCATION** _For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation._ **Minimum Required:** High school diploma or equivalent. Completion of the following courses or the military equivalents: + Medical Terminology + Anatomy & Physiology or Body & function or Human Body in Health & Disease. + Pharmacology **Preferred/Optional:** Graduate of an accredited Medical Assistant program with American Association of Medical Assistants (AAMA) or Registered Medical Assistant (RMA) certified. Associate degree in wellness, health promotion, health education or related field and bilingual capabilities. Completion of military training: Medical Specialist (68W), Hospital Corpsman (HM), Aerospace Medical Service Specialist (4N0X1), Combat Medic (68W), Independent Duty Medical Technician(4NOX1C), Pharmacy Specialist (68Q), Pharmacy Technician (HM-8404, 4T0X1). **EXPERIENCE** **Minimum Required:** Minimum of one-year experience working or volunteering with direct customer service experience in human services, healthcare, community outreach, health education, or related direct service and proficiency with written and verbal communication skills and Microsoft Office Suite. **Preferred/Optional:** Two years’ experience working in health and wellness or related field. Experience working with the needs of the Veteran population. Military service. **CERTIFICATIONS/LICENSES** _The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position._ **Minimum Required:** None **Preferred/Optional:** Current AAMA or RMA certification. CHW Certification. Motivational Interviewing (MI) Certification, Certified Health Coach (CHC) through the National Society of Health Coaches (NSHC); Health Coach Certification through the American Council on Exercise (ACE); Health and Wellness Coach Certification through Well Coaches School of Coaching; or Coach Training Certification through Coaches Training Institute (CTI). This is a hybrid position.
