Job overview:
Provides documentation assistance during a triage process, gets medical information directly from the patient to document in the progress note, adherence to protocols mandated by our medical broad. Medical information includes but is not limited to; Chief complaint, History of Present Illness, Medication Reconciliation, Medical History, Allergies, Hospitalizations, Surgery, Family Medical History and Social History.
Responsibilities:

 Prepares for patient visit by reviewing patient record prior to it (reason of the visit, medical history).  Verify GAP Checker and ensure that all metrics are compliant.  Alerts MA about triage protocols and care gaps.  Establishes a conversation with patient to accurately document in progress note: Chief Complaint, HPI, - -Medications, Medical History, Allergies, Hospitalizations, Surgery, Family Medical History, Social History and ROS.  Adds templates as required for HPI, ROS, Examination and Well Visits.  Change status of the visit to ready for provider.

Qualifications:

 Excellent skills to accurately and completely document patients; medical histories, understanding provider’s instruction, and for accurately documenting patient’s medical information.  Good judgement, organizational ability, initiative, attention to detail and the ability to be self-motivated. Ability to multi-task and work effectively in a high-stress and fast-moving environment.  Competence to work alone and in team, team player setting.  Possess a thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA.  Experience with medical terminology.  Excellent computer and phone skills.  Impeccable verbal, written and interpersonal skills.  Bilingual, fluent in English and Spanish.  Willing to make one-year commitment to work.  Perform other duties as assigned.  Ability to travel as required to perform duties or for training.

Education and experience:

 Medical knowledge.  Basic Word, Excel, PowerPoint.  Excellent redaction skills.  Able to establish a conversation in English and Spanish.
