POSITION SUMMARY:   

CVSHealth is seeking a Nurse Practitioner to join our National Virtual Primary Care team. You will work in collaboration with a dedicated care team to provide evidenced based, primary care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients 18 years of age and older. This full time clinical position is 100% remote supporting patients primarily located in the Pacific, Mountain and Central time zones. The care team will work with NP daily to drive better outcomes for entire panel.

Licensure Requirements:

Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing in at least one of the following states: Washington, Oregon, California, Idaho, Nevada, Arizona, Utah, Montana, Wyoming, Colorado, New Mexico, Alaska, Hawaii, North Dakota, South Dakota, Nebraska, Kansas, Oklahoma, Texas, Minnesota, Indiana, Missouri, Arkansas, Louisiana, Wisconsin, Illinois, Kentucky, Tennessee, Mississippi, Alabama.

PRIMARY DUTIES AND RESPONSIBILITIES:

Responsibilities include but are not limited to

Patient-Centered Quality and Safety

Demonstrates knowledge of etiologies, risk factors, pathophysiology, presentation and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional care patientsPerforms history and physical assessments of patients that are accurate, complete, concise, relevant and appropriate for age, gender and clinical problemEvaluate and treat patients' medical needs with safe and high quality careCounsels and educates patients and familiesDocument all patient care within an electronic health record in line with current policies and proceduresWorks as a member of the care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is provided for all patients and at all timeProvides patient centered care that is , compassionate, appropriate and effective for the promotion of health, prevention of illness and treatment of disease Referral to appropriate Specialists and patient care components as appropriatePossess an awareness and a working understanding of quality measurement in the management of chronic disease conditionsDocument patients that have expressed a desire to join care team panel, using approved EMR workflowsParticipate in/lead practice committees (e.g. Epic Optimization, Care Pathways, etc.)

Communication and Interdisciplinary Collaboration

Collaborates with care team to ensure the delivery of high quality, patient centric, longitudinal careParticipate in daily care team huddles to review patient care including precharting, open care gaps, labs/diagnositics/referral reports, etcCommunicates effectively with members of the care team, other health professionals and health related agenciesWorks effectively with care team membersPromotes mutual respect and trust working within a team modelCultivate and maintain all levels of communication; adjusts communication style to effectively influence quality outcomes and patient needs

EDUCATION: 

Completion of Master’s Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role required

ADDITIONAL REQUIRED SKILLS AND ABILITIES: 

Current National Board Certification, Family PracticeA minimum of 5 years of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practiceEffective verbal, written, and electronic communication skillsOutstanding organizational skills and ability to multi-taskInitiative, problem solving ability, adaptability and flexibilityAbility to work remotely in a care team cultureIs proficient with information management and technologyObtain, and maintain in good standing, multi-state licensuresScheduled on-call rotation to provide after-hours care as needed for VPC members

WORKING ENVIRONMENT:

Virtual Care Team members must meet minimum requirements for remote care delivery, including: broadband connectivity, a quiet setting with a neutral background to conduct visits from, and the ability to uphold patient privacy per CVSH guidelinesWhile performing the duties of the job, the employee is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses Specific vision abilities include the ability to view and read a computer screen and other electronic devices  

Pay Range

The typical pay range for this role is:

$78,331.50 - $196,800.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
