Position Details 

Lead consumer engagement in community settings through the Supported Employment model, driving job development, placement, and coaching.    Conduct vocational assessments and provide tailored skill development to help individuals achieve employment and educational goals.    Collaborate actively with consumers and community partners, participating in Community Support Planning to align support strategies.    Build and maintain relationships with employers, providing ongoing support and guidance to ensure sustained consumer success. 

Our Assertive Community Treatment (ACT) model supports individuals with serious mental illnesses who may not have experienced success or haven't found effective support within traditional outpatient programs. We offer tailored services emphasizing relationship-building and active involvement in enhancing functioning and symptom management.

Services involve weekly home visits, provided either in individuals' residences or within the community, utilizing personal vehicles for client transportation and community-based tasks.
