Responsibilities

Care for individual based on personalized Plan of Care Informs the Director of Nursing and any other appropriate personnel of changes in the client’s conditions and needsCounsels client and family through care process while updating family on a regular basisAssures proper maintenance of clinical records in compliance with local, state, and federal lawsParticipates in agency performance improvement activities and continuously staying informed about changes in the field of nursing and home health care

Requirements

Current unrestricted RN license in the state of Arizona.Minimum one (1) year of documented experience required. One (1) year experience in a home health agency, primary care clinic or health facility, preferred.Reliable car/auto insuranceClean background and criminal recordUnderstands regulatory compliance with state and federal regulationsAdhere to HIPAA and maintain client confidentiality
