Position Overview SCOPE OF ROLE: The Wellness Coach will assist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow up.ESSENTIAL DUTIES & RESPONSIBILITIES:Provide case management to assigned caseloadDevelop and monitor individual service plans in conjunction with agency consumersAssist agency consumers with problem solving and crisis interventionAccompany agency consumers on medical, psychiatric, and public benefit appointments, as necessaryAct as advocate for consumer as appropriate; encourage and support self-advocacyIdentification of any problems in daily living skills; hands on assistance and/or referral in addressing problems in daily living skillsSupport Facilities Management staff in ensuring maintenance of apartment standards as outlined by S:US, OASAS and other diverse fundersSupportive counseling to agency consumers as neededRegular home visits and hospital visits, as necessaryMaintenance of a strong working relationship with all therapeutic support services to ensure the best quality of careDocumentation of all services in compliance with agency standardsAssist with moves with consumers and staff; also include lifting, packing and cleaningWork as a member of the social service team in the development of off-site recreation, socialization, educational and skill building activities with a view towards community inclusion and integrationLiaison to community-based services and groups including civic, social service government, and business organizationsParticipate in ongoing individual and group supervision, team meetings, case conferences, division meetings and trainingsAdhere to on call cell phone system to facilitate crisis intervention, emergencies and other situations that require immediate actionOther duties as assigned Qualifications

REQUIRED EDUCATION AND EXPERIENCE

Bachelor’s degree in Social Work or a related human service degree with two years’ experience working with the people served and/or providing case management2 years’ experience working with individuals with mental illness, substance use issues and/or a history of homelessness or 2 years’ experience working in the human services field requiredExperience in group work, tenant organization, case management, referral, and/or community outreach strongly preferredExperience or training in peer counseling or advocacy preferredWorking knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System
