Summary
The Wellness Nurse plays a key role in health promotion and illness prevention of patient populations through nursing-patient collaboration and patient education. This nurse performs the Medicare Annual Wellness Visit under the direct supervision of a physician (physician on site) for beneficiaries to identify a personalized and individualized prevention plan, complete with objectives and goals for patients as appropriate. Collaborates with physicians for further counseling and treatment as indicated. Maintains knowledge of quality measures set forth by federal programs and ensures adequate documentation in the EHR.
Experience
A minimum of three (3) years relevant clinical experience, preferably in an ambulatory healthcare setting, who demonstrates leadership and autonomy in nursing practice. Preferred experience with care management/utilization review, and payer knowledge. RN considered.
Education
Completion of an accredited LPN program
Licenses
The Wellness Nurse must be licensed in both New York and Pennsylvania. The applicant must have a current license as a Practical Nurse in their state of practice prior to position start date. Additional state licensure must be obtained within 6 months of hire. Patient outreach and contact will be limited to those patients living in the state of current licensure until dual licensure is obtained.
Essential Functions
Promotes wellness and illness prevention using clinical nursing knowledge of disease process, risk factor identification, and illness prevention strategies. Performs all elements of the Medicare AWV with beneficiaries in accordance with Documentation Standards Policy and the AWV Policy and Procedure. Develops a written, age appropriate, individualized prevention plan for each patient including, but not limited to, psychosocial/behavioral risk factors, physiological risk factors, age-appropriate preventative services, recommendations from the USPSTF, and vaccination schedule. Collaborates with physicians and team members to ensure patient receives referrals and follow up care as appropriate. Maintains relevant nursing knowledge of evidenced based strategies related to health promotion and illness prevention, as well as management of chronic conditions. Educates patients and their families regarding disease processes, treatments, risk factor modification strategies, and prevention strategies as appropriate under the nurse’s scope of practice. Maintains thorough knowledge of quality measures related to such evidenced based practices and ensures adequate documentation of metrics in the EHR when appropriate. Performs Nursing functions such as, but not limited to, providing immunizations, and collecting laboratory specimens.Other Duties
Travel for this position may be required. Participates in Quality Data Abstraction and/or validation as needed. Participates with coordination of patient outreach and scheduling activities as needed. Participates in professional development activities and maintains professional affiliations. Maintains confidentiality.
