Wellness Nurse (Part-Time)

Schedule: Day shift (7 a.m. to 3 p.m.), 2 days on weekend and 2 days during the week Pay: We offer competitive pay for LPNs ranging from $33 to $36 per hour! About the Position This position is primarily responsible for monitoring the clinical and holistic health of Assisted Living residents, coordinating necessary services, and delivering care utilizing the nursing process of assessment, planning, implementation and evaluation, while maintaining the standards of professional nursing. The Wellness Nurse provides person-centered care in collaboration with care partners, medication aides and interdisciplinary team. Key Duties + Assign tasks to Care Partners and/or Med Techs in accordance with care plans and individual service plans. + Functions as a clinical resource person coordinates communication with the provider and other ancillary services in order to direct resident care on a designated shift. + Conducts daily wellness checks to monitor, evaluate and treat residents who have had a recent or ongoing change in condition, including collaboration with interdisciplinary team members. + Coordinates resident medical appointments and transportation as requested. + Provides leadership by being a role model and providing highest quality service to customers including residents, internal customers, and partners/suppliers. Job Requirements + Must be a graduate of an approved RN or LPN program. + Must have current licensure in the Commonwealth of Virginia or compact state of residence. + Must havePrefer a minimum of two years of nursing supervisory experience. + Must have current CPR certification. + Must possess ability to deal tactfully with residents, staff and family members.
