Description
THE POSITION:
Under the general supervision of a Program Administrator, the incumbents will support Ambulatory Care licensed and registered professional staff in providing community outreach, education, case coordination and assistance for individuals with complex health conditions involving physical, behavioral, and social needs.
The incumbents will provide field-based services and outreach to at risk populations including patients with health and health-related social needs. The current vacancies are throughout the County of Ventura.

The ideal candidate demonstrates excellent relationship-building, customer service, and public service skills. They will have strong communication and active listening abilities, effectively engaging with individuals and groups from diverse cultural, social and economic backgrounds. They will be compassionate and patient, and possess a genuine desire to assist others. Time management, planning, and organizational skills will be key, along with familiarity with local resources and community service organizations that provide services to at risk populations with health-related social needs. The ideal candidate will be comfortable meeting documentation standards and will have previous case-management knowledge and/or experience. Finally, they will be willing and able to work independently as well as collaboratively within a team.
WHAT WE OFFER
The County of Ventura offers an attractive compensation and benefits package that includes:
+ Market Based Premium Pay (MBPP): $2.50
+ Educational Incentive - An educational incentive of 2.5% for completion of an associate's degree, 3.5% for completion of a bachelor's degree, OR 5% for completion of a graduate's degree.
+ Bilingual Incentive - Incumbents may also be eligible for bilingual incentive depending upon operational need and certification of skill. Proficiency levels by exam are $.69 per hour (Level I), $1.00 per hour (Level II), or $1.32 per hour (Level III).
+ Health Plans – You are afforded an annual flexible credit allowance for purchasing medical, dental, and/or vision insurance from a group of authorized plans.
+ Flexible Spending Accounts - Pre-tax benefit towards eligible medical, dental and vision care expenses.
+ Deferred Compensation - Eligible to participate in the County's 401(k) Shared Savings Plan and/or the Section 457 Plan. If you participate in the County sponsored 401(k) plan, the County will match a part of your contribution up to 3% of your salary, on a per pay period basis. Following five (5) years of continuous County service, an additional 2% non-elective contribution will be made to the 401(k) plan.
+ Pension Plan - Participation in the County's defined benefit pension plan that provides lifetime payments to retirees. Members are "vested" after attaining five years of retirement service credit. If eligible, you may establish reciprocity with other public retirement systems such as CalPERS.
+ Holidays - 12 paid days per year which, includes a scheduled floating holiday.

Community Health Worker is represented by the Service Employees' International Union (SEIU) and is eligible for overtime compensation.
The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies for these positions only . There are currently two (2) vacancies within Health Care Agency - Ambulatory Care.

Examples Of Duties
Duties may include but are not limited to the following:
+ Travels to meet and engage with patients at home and in the community to build trust and assess clients needs, goals, and barriers to accessing care;
+ Helps patients navigate community and healthcare resources, including scheduling medical appointments, connecting to health related social needs, such as housing, food and transportation services, and identifying other social support options;
+ Provides basic, culturally appropriate health education and information within the CHW's training and scope, such as promoting preventive care and accessing community resources;
+ Collaborates with licensed healthcare professionals, such as social workers and nurses, to support care plans and ensure patients have access to needed services;
+ Advocates for patients by assisting them in navigating systems, understanding their rights, and addressing barriers to care.
+ Conducts follow-up visits or phone calls to check on patients' progress, remind them of appointments, and reinforce care plan goals.
+ Accurately documents patient interactions and progress in alignment with organizational protocols and requirements;
+ Meets productivity and quality goals set by agency;
+ Participates in care team huddles, meetings, and required training; and
+ Performs related duties as required.
Typical Qualifications
These are entrance requirements to the exam process and assure neither continuance in the process nor placement on an eligible list.
EDUCATION, TRAINING, and EXPERIENCE
High school diploma, GED or equivalent, PLUS any combination of education and experience which has led to the acquisition of the required knowledge and abilities.
NECESSARY SPECIAL REQUIREMENTS:
+ Willing to work out in the field with at risk populations which may include adolescents, people with disabilities, homeless populations, individuals with chronic illnesses, low-income individuals and families, etc.
+ Must possess or obtain a valid California driver license by date of hire.
+ Some knowledge and/or experience of case management principles.
+ Experience and ability to work in a Windows environment and using Microsoft Word required.
DESIRED:
+ Experience working with diverse populations or in a similar role.
+ College course work in social sciences, ethnic studies, health sciences, communications, or related fields.
+ Bilingual (English/Spanish)
+ Strong communication and listening skills.
+ Knowledge of local community resources and community services organizations that provide services to Ventura County at-risk populations.
+ Obtain a Community Health Worker Certificate from a California Department of Health Care Access and Information approved training program.
+ Previous experience working with relevant Federal (i.e. Social Security) and/or State (i.e., Medi-Cal) Programs or private health insurance companies.

Candidates must earn a score of seventy percent (70%) or higher to qualify for placement on the eligible list. Applicants successfully completing the examination process may be placed on an eligible list for a period of one (1) year.
