As a Home Health Wound and Ostomy Registered Nurse, you will function independently and will be responsible for providing patient care in the home setting.
You will work autonomously with a collaborative and supportive team. This is a multidisciplinary team of medical professionals with various skills, experience, education and specialties all working together through the care management process. This is a team that is there for each other and communicates a lot via phone. They support each other when questions arise and help each other in making sure we provide the best care possible to our patients.
Shift: PRN-Monday-Friday daytime hours
The Work:
Participating with interdisciplinary team, providing clinical guidance to team for complex wounds and ostomies.Provide consultation and direct patient care utilizing best practice.Evaluate wound and ostomy products, collaborating with team for product choice.Participating in QAPI activities including participation in annual survey preparation and survey process, and maintenance of CMS standards.Adhering to all nursing policies and standards, and proactively strives to maintain and improve quality outcomes.Requirement:
Must be a licensed Registered Nurse in Missouri & Kansas. Wound Care certification is required.Why St. Luke’s Home Care:

Accountable for utilizing the nursing process. Responsible for coordinating the care for a group of patients and consulting with other members of the health care team when indicated. Provides nursing care in compliance with the Nurse Practice Act.
