The Wound Care Nurse is responsible for developing and ensuring implementation of plans of care that are comprehensive and patient specific, in collaboration with the Interdisciplinary Team. The Wound Care Nurse will be responsible for establishing an evidence-based Wound Care Program at Terence Cardinal Cooke and serving as an expert in the management of wounds and promotion of education and excellence in this area. Furthermore, the Wound Care Nurse will be a resource for patients and their families, providing comfort and support. He or she will also collaborate with medical and nursing leadership to promote institutional standards of care and monitor relevant data and benchmarks.
Preserving integrity of skin by implementing and monitoring skin prevention strategies.Ensuring all wound and skin care protocols followed. Organize multidisciplinary wound and skin rounds on a weekly basis for high risk patients; patients with current pressure ulcers and other significant wounds.Providing hands-on wound care for all difficult and non-healing wounds and formulating patient-specific care plans.Serves as a resource for facility staff on all issues related to wound and skin care.Provides In-Services and Wound Care Competencies to Care Members on all shifts on an ongoing basis.Participates in the Bi-Annual Skills Fair; assists Staff Development throughout the Skills Fair.Records timely and accurate documentation weekly on tracking; push tools; care plans and Wound Care Notes.Notifies DON/ADON of any facility acquired areas weekly for RCA to be completed.Follows Archcare Wound Care Formulary Products.Develops and implements standards of care, policies, and procedures, particularly in problematic areas or areas of ambiguity.Regularly reviews and assesses facility wound care programs and processes.Provides support, guidance, and counseling as needed to patients and families.Creates monthly reports for QAPI and administration and shares knowledge to improve quality, efficiency, and costs.Submits weekly and monthly Nosocomial Wound Data to the Administrator. RN LicenseWound Care certification
Skill Requirements
Demonstrates advanced clinical level skills and ability to assist others in the achievement of quality care. Demonstrates competency with computers, databases, and software relevant to position requirements.Skill Desired
Understands principles of growth and development in a healthcare organization and is comfortable managing, educating, and monitoring staff. Demonstrates ability to formulate, implement, and evaluate plans for improvement. Is critical in review of cases with relevant staff and is prepared to handle clinically complex cases. SalarySalary: $52.00 - $57.00 per hour
