**HYBRID POSITION - THIS PERSON WILL TRAVEL TO ALL 3 TERRITORIES TO WORK WITH CLINICIANS**
With limited supervision, provides consultation to clients with wounds, fistulas, and ostomies. Utilizing the nursing process and the expertise of Wound Care Nursing, coordinates the total rehabilitative needs of the client. This includes visual assessment to provide recommendations and guidance of care as well as education to the client and his/her significant others and/or caregivers. Instructs nurses, physicians and other health care team members in the care of clients with wounds, fistulas, and ostomies. Performs related supply management and documentation tasks.
Essential Duties and Responsibilities:
Utilizing the nursing process, initiates assessment, planning and nurses implementation of care; also instructs nursing personnel in the care of clients with wounds, fistulas, and ostomies..Assists physicians in caring for clients with wounds, fistulas, and ostomies., teaching self care and consultation. Provides comprehensive post-operative care to clients and teaching to clients and their significant others regarding the care.Evaluates, selects and recommends supplies and equipment for clients with wounds, fistulas, and ostomies.. Documents timely and accurate computer entry of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets and other required patient/employment documents and reports. Attends client care conferences and staff meetings as needed; communicates client's special needs to appropriate health care team members.Serves as a consultant to the nursing staff, physicians and other health care team members related to his/her area of expertise.Provides coordination of the plan of care and maintains continuity of client care with other health professionals assigned to the case. Provides consultation on patient care rendered by Licensed Practical Nurses, Home Health Aides and support service personnel.Develops and monitors standards of care and assists in the formulation and evaluation of procedures, protocols and policies related to his/her area of expertise.Has the ability to perform in person visits weekly for appropriate assessment and implementation of care; as well as any teaching to nursing or patient and family member. Orients new nursing personnel to the role of the Wound Care Nurse.Provides in-service educational programs as needed to nurses, physicians and other health care team members in cooperation with the Supervisor of Clinical Services.Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.Engages in research projects when appropriate.Performs other appropriately assigned duties as required.Minimum Qualifications:
Education: Bachelor's degree from a NLN accredited school of nursing. Satisfactory completion of a WOCN accredited Nursing Education Program. Current board certification in Wound Care Nursing from the WOCN Certification Board (WOCNCB).
