Job Summary:
The incumbent of this position provides high fidelity wraparound services for youth and families, facilitates family choice and voice, and collaborates with key stakeholders, community partners, and team members.
Essential Job Functions:
Ensure positive client experience and best practices:
Facilitate high fidelity wraparound; using up to date policies/procedures and remain updated on the latest trainings related to funding and/or position. Work with site coaches/supervisors and implement feedback.
Tracking and Organization:
Develop an organizational system to manage demands of DMHA requirements, System requirements, and additional funding sources.
Accountability and documentation:
Ensuring documentation is completed timely, accurately, and submitted as required. Maintaining ethical standards of the System and funding sources.
Develop and maintain internal and external partnerships that serve the best interest of our clients and our organization:
Establish positive working relationships both internally and externally, maintain professionalism, and ensure communication across settings. Collaborate with community groups, funders, partners, courts, political leaders and other organizations to help achieve the goals of the organization and ensure stakeholders are aware of our programming and that we obtain referrals in a timely manner.
Management of funding sources/approved services:
Ensure assessments are completed timely, confirm services are approved, load services in EMR system, and relay information to external/internal providers. Manage available units/approved services and any additional follow up. Also have an understanding of the various funding sources and services covered.
Crisis and Safety:
Assessing risk and development of safety plan, report any abuse/neglect, or any other emergency situation. Assist in direct admission for acute care, PRTF/RTF placement, and SOF. Collaboration with courts/legal entities to ensure safety. Demonstrate an overview of our crisis and safety policy, forms, and documentation for both the System and funding sources.
Meet the metrics assigned to this position:
Compliance with internal/external trainings at 100%
100% of documentation must be present in the chart.
Documentation is done in a timely manner 100% of the time as specified by the program. – other ideas 100% of NOAs must be updated prior to expiration date. (ask site coach or look at standards for timeframe). Annuals submitted 30 days prior to expiration.
Notes completed on time 95%
Request monthly reports by the 5th business day of the month (if haven’t already received). Review the monthly report and document the review by five business days from receiving it.
Facilitating and billing 100% of CFTMs
Other duties as assigned by Program Leadership.

Experience and Education Qualifications:
Bachelor’s Degree required
2 years experience with SED population or received clinical supervision by licensed provider.

Employment Requirements:
Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
Completion of New Hire Orientation at the beginning of employment.
All training requirements including Relias at the beginning of employment and annually thereafter.
Current driver’s license, acceptable driving record and current auto insurance.
Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
