Youth Advocate

Position Overview: 

The Youth Advocate conducts weekly meetings with youth and generate progress notes weekly, conducts assessments for youth upon entry to the program and every 6 months, completes monthly outcomes scales for youth, attend monthly network meetings in the community, facilitate independent living skill workshops with youth, help youth obtain vital documents, keep youths live charts organized and ready for audit purposes, and provide 6-month aftercare to youth when they transition out of program. 

Position Qualifications:

Bachelor’s in social work or 3 years related human services field with social work experience or case management or, HSD and 5 years of experience working with at risk youth. Experience in child welfare or work with older adolescents. Ability to function successfully on interdisciplinary treatment team. NYS Driver’s license required. Excellent written and verbal skills including ability to successfully manage multiple tasks.
