Youth Services Clinical Well Being Manager
Salary: $65,000/year POSITION OBJECTIVE: This position manages clinical well-being activities for youth and young adults ages 17 and older in the four-county area. The Well Being Manager will be a single point of contact for youth, case managers, and community treatment providers offering information and direct referrals to address the well-being needs of youth transitioning out of care. This position coordinates community mental health, clinical and substance abuse treatment services and discharge planning for any youth and young adult hospitalized or admitted to an inpatient facility. ESSENTIAL FUNCTIONS: Develop and maintain good working relationships throughout the FPOCF System of Care with providers specializing in Mental Health, Behavioral Health, and Substance Abuse treatment. Identify gaps in provider network, coordinate county service needs with provider accessibility, and communicate findings to the Youth Services Administrator to expand network and build provider relationships. Provide clinical insight, information, and referrals for case managers, caregivers and youth/young adults to available programs and services in the community. Review clinical reports including CBHA’s, Psychological Evaluations, Psychoeducational Evaluations, and others to provide information and recommendations to assist in successful transition. Facilitate MDT staffings as needed for young adults. Collaborate with the FPOCF Utilization Management and APD lead to finalize APD services for young adults. Review and assist with finalizing Regis Little Guardianship Recommendations. Coordinate services for young adults after Baker Acts. Provide guidance on Medicaid, Social Security, and APD eligibility for young adults. Ensure all services are transferred to young adults at age 18. Provide after-hours consultation & direction related to youth stabilization as needed. Conduct presentations in the community regarding the needs of transitioning youth. Offer workshops and one-on-one consultation to case managers, caregivers and youth on mental well being best practices. Act as the FPOCF subject matter expert regarding mental well being needs for youth and young adults exiting foster care and participate in community-wide mental health initiatives. Assist in obtaining feedback from stakeholders to evaluate services and implement changes as needed. Record statistical data and assist in contract reporting requirements. Participates in creation and annual reviews of Unit Policy and Procedure Manuals to ensure all policy and procedures reflect current state, federal and system of care requirements. Participate in the agency strategic plan & quality improvement processes. Ability to safely & successfully perform essential job functions consistent with the ADA, FMLA, & other federal, state, & local standards, including meeting qualitative & quantitative productivity standards. Ability to maintain reasonably regular, punctual attendance consistent with ADA, FMLA & other federal, state, & local standards. Ability to comply with all personnel policies & procedures. QUALIFICATIONS: REQUIRED: Master’s Degree from an accredited college or university in social work or mental health counseling Minimum of two years of experience in mental health services or social service-related field Case management experience and/or knowledge of services in the community that are available to the young adult population. PREFERRED: Experience working with at risk populations. Licensure preferred as LMHC, LCSW or LMFT Clinical expertise in continuity of care and service provision to include experience with sexual safety and administration of psychotropic medication. Knowledge of the funding & provider resources available in the county and surrounding area Familiar with resources/services supports available for children with substance abuse, mental health, developmental disabilities, and delinquency issues. SKILLS: Excellent organization and communication skills and highly self-motivated to work independently. Ability to build relationships and work collaboratively with other professionals and non-professionals. Highly sensitive to the needs of youth transitioning out of care. Ability to organize information and workload to accomplish tasks quickly and accurately. Must be proficient in Word, Excel, Power Point and Outlook. Demonstrate cultural and linguistic competence & sensitivity to population served. Ability to travel. SPECIAL CONDITIONS: Thorough Background Screening upon hire including FBI, FDLE, Local Law check, Employment Reference check, DMV check, Criminal Records check, Social Security Trace & Drug Screening. FDLE check will be run at least every 5 years.
